Support Item Fees:

The prices for the specific Services that we provide to you are set out in the NDIS Pricing Arrangements and Price Limits. All prices are subject to change when the NDIS Pricing Arrangements and Price Limits changes. The prices we charge you will be automatically adjusted to reflect the prices outlined in the most up to date NDIS Pricing Arrangements and Price Limits and the specific Services we perform.

Fees will be charged in accordance with NDIS Pricing Arrangements and Limits for Group and Centre Based Activities as follows:

i.             Hourly fee charged will be dependent on day of the week the participant attends the activity i.e. Weekday, Saturday, Sunday or Public Holiday;

ii.            Hourly fee charged will be dependent on the ratio of supports the participant is provided i.e. from 1:1 through to 1:5; and

A Centre Capital Cost Fee will be charged on an hourly as per the NDIS Pricing Arrangements and Limits.

Terms of Agreement

1) This Agreement is made in accordance with the rules and the goals of the NDIS and for the purpose of providing the Services to you in accordance with your Plan.

2) Please inform our staff if the participant has any of the following which may impact the delivery of activities:

• heart condition impacting engagement in physical activity
• seizures or severe respiratory conditions e.g. asthma
• severe allergies or phobias 
• absconding (running away)
• any other matters we should be aware of. 

3) You agree that we may charge you 100% of the relevant amount that would otherwise be payable to you if you do not show for a scheduled service, or you have given us less than 24 hours’ notice of cancellation.

4) The participant (client, you) and its representative release and indemnify Brisbane Basketball Incorporated (on a full indemnity basis) in respect of all liability for any loss, claim, damage, penalty, fine, or expense in connection with Brisbane Basketball Incorporated provision of the services to the participant including, but not limited to, liability arising from the death or personal injury of the participant or any other person and any indirect or consequential damage or loss suffered by the participant, the participants representative, or a third party, arising from any act or omission by Brisbane Basketball Incorporated or any Brisbane Basketball Incorporated director, employee, agent, officer, contractor, or third party.
